Sorry, but I’m not quite clear on what you need to be approved. I’ve looked at that form, and it would seem to be a form for registering your own custom nameservers for a .DK domain. We don’t offer .DK domain registrations or transfers at this time, so you’d want to contact your current domain registrar with help on that form.
