"Ownership Information not supplied" Problem

I have a premium SSL certificate installed but I get the following message when I try to see the identity information using firefox.
Web Site Identity
Website: http://www.madebyus4u.com
Owner: This web site does not supply ownership information.
Verified by: Not specified
and under that I get the following info.
Technical Details
Connection Partially Encrypted
Parts of the page you are viewing were not encrypted before being transmitted over the Internet.
Information sent over the Internet with out being encrypted can be seen by other people while in transit.
I have read this forum page on how to solve the second issue,

but for the first I also read this one,

Esentially the page above tells me that the problem with your site not being identified even though the SSL certificate is not installed is because the SSL certificate is not a Deluxe or a Premium one.
In my case however I have premium SSL certificate so what might be the problem.
Note: I did supply all the required information about the company while generating my CRS.

You need to address the issue with the partially encrypted connection in order for the address bar to display the identification details for your site. If you check a page that does not have this issue, you will see that the details are displayed as expected. This URL is an example of that:
